What do I need to file my tax return:
Depending on your situation you will need a variety of documents. In addition to our tax questionnaire here are some common items you will need.
Income source documents:
- W2 – Wages and salary
- W-2G – Gambling winnings
- 1099-R –Retirement Income
- 1099-INT – Interest
- 1099-DIV – Dividends
- 1099-MISC – Miscellaneous income, including self-employment
- 1099-G – Government payments
- 1099-SA – MSA or HSA distributions
- 1099-B – Stock exchanges
- SSA-1099 – Social Security benefits
- Schedule K-1 – Partnership income or loss
- Alimony received
Documents for items you have paid during the year:
- 1098 – Mortgage interest
- 1098-E – Student loan interest
- 1098-T – Tuition
- 1095 – Health coverage
- Alimony paid
Records and receipts to document:
- Charitable donations
- Health care expenses
- Mileage reports
- Child care expenses
